Heart-rate variability is the gap between consecutive beats, and it varies more than people expect. The apps below differ less in what they measure than in how much they tell you it means.

Passive versus active measurement
This is the split that matters most and it is rarely made explicit. Passive apps read HRV your wearable already recorded into Apple Health, so your entire history is available the moment you install. Active apps have you take a measurement — often a morning reading with the phone camera — which is more controlled but starts your dataset from zero and depends on you doing it consistently.
Neither is better in the abstract. Active measurement under identical morning conditions is more comparable day to day; passive measurement is the one you will still be using in six months.

Far and away the most established app here, correlating HRV-derived stress against sleep, work and workouts. Best for: people who want the deepest read on their own physiology. Limitation: subscription, learning curve, and context is inferred rather than read from a calendar.
Elite HRV — the free option
Free, 4.7 stars from nearly 6,900 ratings, iOS 13 or later — the widest device support here. States Apple Health integration explicitly alongside Training Peaks, Strava and others, so it fits into an existing training stack. Best for: HRV tracking without a subscription. Limitation: assumes you want to learn what the numbers mean, and there is no Apple Watch app.
Athlytic — HRV as a recovery score
Free with a subscription, 4.8 stars from nearly 11,000 ratings, iOS 17 or later. Converts Apple Watch data into daily recovery and strain scores in the style of a dedicated fitness wearable. Best for: athletes who want a single actionable number each morning. Limitation: if you are not training regularly the scores have little to work with, and a single score hides the reasoning behind it.
HRV4Training — camera-based, paid once
$9.99 once, 3.6 stars from 288 ratings, iOS 15.6 or later. Active camera measurement, no subscription. Best for: people who want controlled morning readings and refuse recurring costs. Limitations: its App Store listing does not mention Apple Health at all, so verify that before buying if passive reading matters to you. It has the lowest rating here, and no Apple Watch app.

A caution about single scores
HRV is genuinely informative in aggregate and genuinely noisy day to day. Caffeine, alcohol, a warm room, poor sleep, illness and the time you took the reading all move it. Trends across weeks mean something; one morning number usually does not. Any app converting a single reading into a confident verdict about your readiness is being more certain than the underlying signal supports.

What is a good HRV score?
There is no universal good value — HRV varies enormously between individuals with age, fitness and genetics, and a figure that is high for one person is low for another. What is meaningful is your own trend over weeks against your own baseline, not a comparison to anyone else.
